Your story is not untypical. Just keep going back to the 12 Step programs and you will make it. Use your sponsor a lot. Call him everyday in the beginning, the first several months. Talk with him about the steps.

I have been to 3 rehabs and still continue to drink. I go to AA and I have a sponsor. I can get some clean time and then just seem to have to have a drink and it of course ruins eveything. I guess i am one of the lost ones.
